The Digestion & Intestinal Card combines a comprehensive blend of digestive enzymes (pineapple extract providing bromelain, papaya extract providing papain, and DigeZyme®), multi-strain probiotics (Bifidobacterium and Lactobacillus species), the prebiotic fructooligosaccharides (FOS), curcumin with BioPerine®, fenugreek, L-glutamine, vitamin C, and citrus bioflavonoids. This report reviews the available scientific evidence supporting the potential contribution of these ingredients to digestive comfort, intestinal barrier function, gut microbiota balance, and overall gastrointestinal health.

Probiotics and Prebiotics for Gut Health

Probiotics are live microorganisms that, when consumed in adequate amounts, confer health benefits by helping maintain or restore a balanced intestinal microbiome [3][6]. Prebiotics are non-digestible dietary compounds that selectively stimulate the growth and activity of beneficial gut bacteria, thereby supporting gastrointestinal health [2][3].

A randomized controlled clinical trial involving 100 adults with gastrointestinal dysfunction evaluated two high-potency multi-strain probiotic formulations delivering 1,200 billion CFU/day and 2,000 billion CFU/day, respectively. Additional evidence comes from a comprehensive narrative review of 22 randomized controlled trials, which concluded that prebiotic supplementation consistently increased populations of Bifidobacterium and Lactobacillus, enhanced short-chain fatty acid production, improved stool frequency and consistency, and strengthened intestinal barrier integrity [2].

A systematic review and meta-analysis of 24 clinical trials further demonstrated that probiotic and synbiotic supplementation significantly reduced circulating lipopolysaccharide (LPS) (SMD: -0.54) and zonulin (SMD: -0.49), suggesting improvements in intestinal permeability, commonly referred to as "leaky gut." Digestive Enzymes

Digestive enzymes facilitate the breakdown of nutrients and may also influence the composition of the gut microbiota.

Experimental research investigating the cysteine proteases bromelain (derived from pineapple) and papain (derived from papaya) demonstrated beneficial effects on digestive function and microbial composition. In a study involving healthy C57BL/6 mice, supplementation with 1 mg/day of bromelain or papain for three days reduced the abundance of Proteobacteria while increasing the beneficial bacterium Akkermansia muciniphila. Both enzymes also enhanced pancreatic trypsin activity, while bromelain increased the thickness of the ileal mucosa [4].

An accompanying in vitro study using a reconstructed three-dimensional human intestinal tissue model found that very high enzyme concentrations (10 mg/mL) increased paracellular permeability by temporarily altering epithelial integrity. These findings suggest that digestive enzymes may enhance nutrient transport at appropriate doses, while highlighting the importance of using clinically appropriate supplementation levels [4].

Additional evidence from a review of 13 studies suggests that supplemental proteases and lipases may also exhibit prebiotic-like effects by enriching beneficial bacterial populations and contributing to a healthier gut microbiota composition [9].

Curcumin, Fenugreek, and BioPerine®

Curcumin is widely recognised for its anti-inflammatory properties and has been investigated as a complementary nutritional strategy for inflammatory gastrointestinal disorders, including inflammatory bowel disease and ulcerative colitis [10].

Systematic reviews and meta-analyses evaluating curcumin supplementation have explored its potential role in inducing and maintaining remission in ulcerative colitis, suggesting beneficial effects on intestinal inflammation when used alongside standard therapy [5].

Because curcumin is naturally absorbed poorly, formulations commonly combine it with BioPerine® (black pepper extract), which enhances its bioavailability and may improve its physiological effects [5].

Although fenugreek is included in the Digestion & Intestinal Card, the scientific literature reviewed for this report does not provide specific evidence regarding its direct contribution to digestive or intestinal health within this particular formulation.
